Get the most out of your Physical Therapy
Do I need a referral for PT?
No! in the state of Colorado, no referral is needed to see a Physical Therapist. You are always welcome to contact your primary care physician first, but it’s not necessary.
What conditions do you treat?
We primarily treat orthopedic conditions from head to toe. We also can do preventative or recovery sessions for those who want to feel their best. As well as wellness PT sessions which are holistic in nature and focus on overall well-being.

How to prepare for your initial evaluation?
Once you have an initial evaluation scheduled, follow the steps below to prepare for your appointment
STEP ONE: Register Your PtEverywhere Account
After scheduling your evaluation, you’ll receive a confirmation email from Summit Physio with instructions to create your PtEverywhere account—our secure patient portal. Your registration link is valid for 7 days, so be sure to complete this step promptly. If you don’t see the email, check your spam folder or contact us for assistance. If your link has expired, we’re happy to send a new one.
STEP TWO: Complete Your Intake Forms
Once your account is set up, log into PtEverywhere and navigate to the “Documents” tab to complete your intake forms. These must be submitted before your appointment so your provider can review them in advance. You’ll also be asked to enter a credit card or HSA/FSA card as part of our intake policy.

Initial Evaluation: What to Expect from Your Physical Therapy Session
What happens during the evaluation?
During your initial physical therapy evaluation at Summit Physio & Performance in Colorado Springs, your performance physical therapist will map your symptoms on a body chart, review your detailed medical history, and conduct a one-on-one assessment. This evaluation is tailored to active individuals, including CrossFit athletes, and includes range of motion testing, strength and flexibility assessments, orthopedic special tests, and functional movement screening to identify the root cause of your pain or limitation.
Will I receive treatment on day one?
Yes, patients begin hands-on treatment during the first visit. This may include dry needling, cupping (myofascial decompression), soft tissue therapy, joint mobilization, or a combination of evidence-based techniques. You’ll also receive a customized home exercise program to support your recovery and performance goals.
What should I wear?
Please wear athletic clothing that allows you to move freely and gives your physical therapist access to your arms, torso, and legs. Ideal attire includes athletic shorts and a sports bra if needed, especially for undergoing shoulder, hip, or back assessments.
What’s next?
After your first visit, you’ll have a clearer understanding of what’s contributing to your pain, mobility issue, or performance limitation. You’ll leave with a personalized home exercise program created by your physical therapist, delivered through the MedBridge platform. Following this plan consistently is key to achieving the best results.

How do I access my home exercise program?
Your physical therapist will send your custom program via text and email with instructions to set up your MedBridge Go app account. You’ll also receive a PDF or printed version of your exercises. If you have any trouble accessing your program, don’t hesitate to reach out—we’re here to help.
Your physical therapist will recommend a plan for the number and frequency of follow-up sessions based on your goals. You can schedule your next appointment directly in the clinic or use the “Schedule Now” button on the left to book online at your convenience.
